Stay safe and healthy abroad
You may need travel vaccinations depending on the country or countries you intend to visit.
To help us advise you on the protection you need, please complete the attached questionnaire and return to the surgery as soon as possible. Ideally this should be at least 8 weeks before departure so that we can arrange a schedule for any required immunisations.
This is important as some vaccines do not confer adequate protection under 14 days from administration, and others up to a year particularly if you intend to travel for a long time and/or to remote areas.
If you are currently pregnant or planning a pregnancy, please inform the Practice Nurse before commencing any vaccinations or receiving advice about malaria protection.
Please allow one week for the processing of this form – we will contact you to make an appropriately times appointment
If you are travelling abroad you may need to be vaccinated. Please make an appointment with the practice nurse to discuss the vaccinations you will need, which will depend on where you are travelling.

The vaccinations recommended by your Nurse are based on travel advice at the time of your request.
Many countries have vaccines ‘sometimes recommended’ which can cause variations in schedules.
If the information you supply to us is incomplete or inaccurate, our recommendations may not be accurate.
PLEASE BE AWARE THAT VACCINES MAY NOT PROVIDE 100% COVER AND THE PREVENTATIVE ADVICE GIVEN TO YOU SHOULD ALWAYS BE FOLLOWED
Travel vaccination fees
Some travel vaccines are ordered on a private prescription which cost more than the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Some malaria tablets are not available for purchase at your pharmacy counter and will be issued on a private prescription. An admin charge of £ 20.00 will be made by the practice for this prescription and you will need to pay the pharmacy for the cost of the tablets separately.
