How to register

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

You can register by:

Please note that when you register with our practice, you will be allocated to either our Little Common or Old Town site, depending on clinical capacity and service availability.

While your NHS registration confirmation may mention “Little Common Surgery,” this refers to the practice as a whole and does not guarantee allocation to that specific site.

We are one practice operating across two locations, and you will be able to access services at both sites as appropriate.

You will have a named, accountable doctor who is responsible for coordinating your care.

We recommend that new patients undertake a health check with a practice nurse.

Nursing and Care Home patients

If you are registering a patient who has recently moved in to a Nursing or Care Home, please do not use the online form and please contact the Practice instead, as additional information is required for registration purposes.

Temporary patients

If you are not registered with the surgery you will be unable to see one of our doctors and you will need to call NHS 111 who can provide advice as to where to go for care and support.

If you are not registered but need treatment at the surgery you will need to complete a temporary registration form.

You can be registered as a temporary patient for up to three months. This means you can be registered with our practice and remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with our practice.

If English is not your first language and you require translation or interpreting services please let us know and this can be arranged at no cost to you.
